All our professionals stay abreast of changing treatments and resources, attending ongoing education programs so they can provide you with the most current education and treatment available.
Program requirements and registration
CDS is required to have a written referral from the physician treating your diabetes. We can obtain this for you if you call us at 1.888.966.6909 with your request and physician contact information. Or, if you prefer, you can ask your physician to forward it to us by mailing it to CDS, 10061 Amberwood Road, Fort Myers, FL 33913; or faxing it to 1.800.529.0543. Click here for the referral form.
