Each participant receives an individual patient manual.
Family members are encouraged to attend.
Our goal is to create a "circle of care" that includes the patient, physician,
managed care company, diabetes educators and the supply company - all working
together to maintain patient compliance and achieve more positive patient outcomes.
CDS program characteristics:
The CDS Diabetes Self-Management Education Program is overseen by a Medical Advisory Board
comprised of certified diabetes educators, registered nurses, registered dietitians, endocrinologists, Internal Medicine physicians and consumers with diabetes.
CDS instructors include certified diabetes educators, with support from registered
dietitians for diet preparation. Many of the CDS educators hold multiple certifications
as registered nurses, registered dietitians and certified diabetes instructors.
As a national healthcare company with multiple site locations, CDS can deliver
its program in all major U.S. markets commensurate with health plan demand.
CDS education is delivered locally by specially trained certified diabetes educators.
The site location is flexible based on the needs of the member population and health plan.
CDS is a sought-after outsourced education provider for many hospitals and clinics.
We can deliver educational programs in health plan- or employer-preferred site locations,
such as schools and offices, for additional member convenience.
CDS provides educational programs in many under-served markets where diabetes education
is not currently available, and is currently one of the largest diabetes education programs
in the states of Florida and Texas.
CDS is a Medicare / Medicaid approved education provider.
CDS offers the CDS Care Coach Member Support/Supply Program designed to provide
ongoing diabetes self-management education and support through telephonic interaction
with the member every 90 days.
