Recognizing how important education and support programs are to diabetes self management and,
in turn, to controlling healthcare costs and maximizing the value of every healthcare
investment, CDS has partnered with many managed care organizations to offer specially
designed programs to assist their health plan members who have diabetes. CDS programs
will assist your members with the
proper management of their diabetes, thus reducing complications and associated expense.
CDS Diabetes Self-Management Education Program
The CDS Diabetes Self-Management Education Program is a comprehensive workshop that teaches
people with diabetes to better manage their diabetes and maintain a better quality of life.
Understanding the diabetes process will allow the patient to monitor and respond appropriately
to daily fluctuations of blood glucose, thus maintaining better control over the disease process.
Taught by certified diabetes educators, registered dietitians and registered nurses, the workshop
is scheduled for two five-hour sessions. The Program includes resource materials and a
free Ascensia blood glucose meter from Bayer Healthcare.
CDS is proud to be one of the largest ADA Recognized Diabetes Education Providers in the nation.

CDS Member Support and Supply Program
The CDS Member Support and Supply Program, available nationwide, is a continuation
of our Diabetes Education. Focusing on establishing an ongoing relationship with
health plan members, it provides them with access to the CDS clinical staff through
the following components:
Telephonic health reminder surveys
Tailored education
Self-management supplies, delivered every 90 days
Quarterly newsletter
